Photo Release Form for Minors

I give Pamper Me Pretti Kids Mobile Spa, LLC, permission to use my child's likeness, image,

voice, and appearances as such may be embodied in any pictures, photos, video recordings,

digital images, and the like, taken or made on behalf of Pamper Me Pretti Kids Mobile Spa,

LLC, party activities. I agree that Pamper Me Pretti Kids Mobile Spa LLC, has complete

ownership of such pictures, etc., including the entire copyright, and may use them for any

purpose consistent with Pamper Me Pretti Kids Mobile Spa, LLC business. These uses include

but are not limited to reprints, publications, advertisements, social media, and any promotional

materials in any medium now known or later developed, including the internet. I acknowledge

that I will not receive any compensation for using such pictures, etc., and as a result of this

release Pamper Me Pretti Kids Mobile Spa, LLC, from all claims that are in any way connected with such

use. I have read and understood this consent and release.

Makeup Consent

As part of the Pamper Me Pretti Kids Mobile Spa, LLC party experience, age-appropriate

makeup, and spa treatment products will be applied. Pamper Me Pretti Kids Mobile Spa parties

are for entertainment purposes only. We exercise particular care in sanitary and use mild kid-

friendly products susceptible to young skin. We make no claims of being licensed professional

technicians, cosmetologists, or estheticians. Therefore, we take no responsibility for any allergic

reactions of any party guest. It is the responsibility of the parent or guardian hosting the party to

alert Pamper Me Pretti Kids Mobile Spa in advance of any allergies that any of the children may

have. Pamper Me Pretti Kids Mobile Spa reserves the right to refuse services on any child for

any reason that we feel may impose a health risk to others. I have read and understood this

consent and release.

Food Allergy

Your child will participate in various activities at the Pamper Me Pretti Kids Mobile Spa.

Additionally, some activities involve food products as part of the experience. Pamper Me Pretti

Kids Mobile Spa takes every precaution to ensure that our girls are safe. Pamper Me Pretti Kids

Mobile Spa, LLC, will not be responsible for any allergic reactions or life-threatening incidents,

including death that could occur. If your child has any known food allergies that may prevent her

from participating in these activities, we will provide an alternative option per your request. I

have read and understood this consent and release. By checking the box and booking a party with

Pamper Me Pretti Kids Mobile Spa, LLC confirms, I have read and understood this consent and

release and assume all responsibility as set herein.

 

Liability

We strictly provide entertainment. Please be aware that children need to be supervised. We

cannot be held responsible for accidental damage caused by your child. Pamper Me Pretti Kids

Mobile Spa, LLC assumes no responsibility or liability for accidents or damage caused during

the party. In the event of damage or loss of our inventory and/or damage or loss to the party bus,

the responsibility lies entirely on the client and or guardian. Pamper Me Pretti Kids Mobile Spa,

LLC is not responsible for allergic reactions and/or any medical injuries from the food,

beverages, wearing costumes, or any activity occurring before, during, or after the party. I hereby

release and agree to hold Pamper Me Pretti Kids Mobile Spa, LLC harmless from and waive on

behalf of myself and my heirs. Any personal representatives any causes of action, claims,

demands, damages, costs, expenses, and compensation for damage or loss to myself and/or

property that may be caused by any act or that may otherwise arise in any way in connection

with any services received from Pamper Me Pretti Kids Mobile Spa, LLC. I understand that this

release discharges Pamper Me Pretti Kids Mobile Spa, LLC from any liability or claim that I, my

family, or any personal representatives may have against the business concerning any bodily

injury, illness, death, medical treatment, or property damage that may arise from, or in

connection to, any services received from Pamper Me Pretti Kids Mobile Spa, LLC. This

liability waiver and release extends to the Pamper Me Pretti Kids Mobile Spa, LLC, with all

owners, partners, and employees.

Coronavirus/COVID-19

I acknowledge the contagious nature of the Coronavirus/COVID-19 and that the CDC and many

other public health authorities still recommend practicing social distancing. I further

acknowledge that Pamper Me Pretti Kids Mobile Spa, LLC has put preventative measures to

reduce the spread of the Coronavirus/COVID-19. I further acknowledge that Pamper Me Pretti

Kids Mobile Spa, LLC cannot guarantee that I will not become infected with the

Coronavirus/Covid-19. I understand that the risk of becoming exposed to and infected by the

Coronavirus/COVID-19 may result from the actions, omissions, or negligence of myself and

others, including, but not limited to, Party staff and other guest and their families. I voluntarily

seek services provided by Pamper Me Pretti Kids Mobile Spa, LLC, and acknowledge that I am

increasing my risk of exposure to the Coronavirus/COVID-19. I acknowledge that my family and

I must comply with all set procedures to reduce the spread while attending a Pamper Me Pretti

Kids Mobile Spa, LLC event.
